Effective Date: August 1, 2026‍

I understand that participation in the LimitlessXO Exosome Therapy Session Giveaway requires a willingness to cooperate reasonably with LimitlessXO and its authorized social media and production team in documenting my experience if I am selected.

If selected, I agree to:

• Participate in reasonable photography, audio recording, video recording, interviews, testimonials, and treatment-progress updates before, during, and after the therapy;
• Be reasonably available for follow-up filming and communications for up to 12 months after my final therapy session;
• Discuss scheduling, privacy preferences, and the proposed use of recordings with LimitlessXO before filming; and
• Review and sign a separate, detailed media authorization and treatment consent before any therapy or recording occurs.

I understand that checking the agreement box does not itself authorize publication of my image, voice, medical information, or testimonial. Any authorization for recording, editing, advertising, social media publication, or disclosure of health information will be addressed in a separate written agreement.

I understand that I may decline to sign the separate media authorization, but doing so may make me ineligible to receive the giveaway prize. Declining media participation will not affect my ability to purchase or receive services from LimitlessXO and its partners outside this giveaway.
